All incoming 9th graders and all new students must have a WWMS/MHS Physical Examination form completed by the time the first day of school.

Any Munster High School student who wishes to participate in a sport must also have the following documents completed before he/she can participate in a sport (including try-outs):

ISHAA Pre-Participation Physical Evaluation Form
Athlete’s Medical and Injury Information

The State of Indiana mandates that all senior students have an updated immunization record on file or a current written medical or religious objection form  on or before the first day of school.

**The ISHAA physical must be completed for athletics.  Every year there after the high school student participating in sports will need an ISHAA Physical and the Medical and Injury Information forms.  The middle school only requires the WWMS and MHS physical for participation in sports.**
